Participant Waiver & Media Release
Fellowship Camp & Conference Center

I understand that participation in Red Letter Day activities is voluntary and that I may choose which activities I wish to participate in throughout the day.

Participation & Personal Responsibility

I understand that some activities offered during Red Letter Day may involve physical activity and may include inherent risks. I accept responsibility for deciding whether an activity is appropriate for me.

I agree to:

• Participate only in activities that are within my normal level of comfort and ability.

• Consider my own physical condition, health, mobility, and other personal limitations when deciding whether to participate.

• Ask questions or seek assistance if I am uncertain about an activity.

• Stop or decline participation in any activity if I believe continuing would be uncomfortable or unsafe for me.

• Follow reasonable instructions and safety guidelines provided by Fellowship Camp & Conference Center staff and event leaders.

I understand that I am never required to participate in an activity and may choose an alternative or simply observe.

By signing below, I acknowledge my voluntary participation and accept the ordinary and inherent risks associated with the activities I choose to participate in. I release Fellowship Camp & Conference Center, Camping Ministries of the Carolinas, Inc., and their employees, volunteers, and representatives from claims arising from my voluntary participation, except to the extent that such a release is prohibited by law.

Photo & Media Release

I understand that photographs and/or video may be taken during Red Letter Day. I give permission for images or recordings in which I appear to be used by Fellowship Camp & Conference Center and Camping Ministries of the Carolinas, Inc. for purposes including newsletters, websites, social media, promotional materials, and other communications related to the organization's ministry and programs.

Media preference:

I will make know to camp staff my preference to be included/excluded from any photos or other media during the day.  I understand that Fellowship Camp & Conference Center will make reasonable efforts to honor my desire to be included in any photos, videos, etc.

Emergency Contact

I will bring with me to the event information on my emergency contact.
