Informed Participation/Assumption of Risk and Waviers & Release

I acknowledge and agree that any use of Fellowship Camp & Conference Center, a ministry site of Camping Ministries of the Carolinas, Inc.'s (CMC's) facilities, services, equipment and premises and my participation in the Red Letter Day programs and activities comes with inherent risks.  I agree that I am voluntarily participating in these activities and will only engage in activities within my physical, social, an emotional comfort levels. I voluntarily accept and assume full responsibility for these risks as well as any and all other risks of the use of Facilities and participation in programs. I agree that I have full knowledge of the nature and extent of all such risks and am not relying on all such risks being described in this document.

In consideration of the use of Facilities and participation in Programs I agree that CMC, its officers, directors, agents, employees, volunteers, insurers and representatives will not be liable for any personal injury, property damage, disability, death, sickness or disease incurred by myself, my family members, dependents, or guests, including minors, however occurring. I understand that I will be solely responsible for any loss or damage, including personal injury, property damage, disability, death, sickness or disease sustained from the use of Facilities and participation in Programs.

I hearby grant permission for Fellowship Camp and Conference Center's staff, officers, volunteers and other leadership to seek medical treatment for me in case of accident, illness, or injury. 

Media, Photo & Video Release

I grant permission to CMC and all event associated parties to use my name and photographs or any other record of my participation in this event for any purpose.  Any exception to this Media, Photo and Video Release must be communicated in writing to CMC staff.
